1. Not establishing a great enough sense of urgency

2. Not creating a powerful enough coalition

3. Lack of vision

4. Not communicating vision properly

5. Not removing obstacles to the new vision

6. No systematic planning and not milestone(or small wins)

7. Declaring victory too early

8. Not implementing changes in the corporation culture.
